Endometriosis Surgery in Bangalore
Surgery for endometriosis removes or destroys the patches of endometriosis, the cysts they cause and the scar tissue that binds organs together. It is usually advised when medicines have not controlled pain, when the disease is extensive, or when it is affecting fertility. Medicines are normally the first treatment, so surgery is a decision to make after a proper diagnosis and a conversation about what you want from treatment. At Sai Speciality Center in Kasturi Nagar, Bengaluru, endometriosis is assessed by Dr. Kavitha Lakshmi Easwaran, MBBS, MD (OBG), who explains the options, including the cases where surgery is not the right next step.
Quick answers
- Is surgery the first treatment? No. Painkillers and hormonal medicines are usually tried first.
- How is it done? Most often by laparoscopy (keyhole), through small cuts with a camera. Larger or deeper disease may need a bigger operation.
- Will it get rid of endometriosis for good? Not always. Surgery can reduce pain a great deal, but symptoms can return and further surgery may be needed.
- Does it help fertility? It can help some women, and it can also affect the ovaries. This is worth discussing before the operation.
- Is a hysterectomy needed? Only in selected cases, usually when the family is complete and other treatment has failed.
When Is Endometriosis Surgery Advised?
Surgery is considered when:
- Medicines have not controlled pain, or are not suitable for you
- Endometriosis is extensive or has formed ovarian cysts (endometriomas)
- The disease is affecting the bowel, bladder or ureters
- Endometriosis is thought to be the reason you have not conceived
- The diagnosis is uncertain and a laparoscopy is needed to confirm it
If you have only mild symptoms that settle with medicines, surgery is rarely needed. For the full picture of diagnosis and medical care, see our page on endometriosis treatment.
Medicines First, in Most Cases
The usual first steps are painkillers such as paracetamol or ibuprofen and hormonal treatment, such as the combined contraceptive pill or other medicines that change hormone levels. Hormonal medicines are not given to women who are trying to conceive, which is one reason fertility plans shape the decision. Pain, tiredness and low mood can also be supported without surgery. If medicines work well for you, there is no need to hurry into an operation.
Types of Endometriosis Surgery
- Diagnostic laparoscopy: a camera is passed through a small cut in the tummy to look for endometriosis. Patches can often be treated during the same operation, so the consent discussion should cover this beforehand.
- Removal of endometriosis patches: the surgeon cuts out or treats the visible disease and frees organs stuck together by scar tissue (adhesions).
- Removal of endometriomas: chocolate cysts on the ovary are removed, with care to protect the healthy ovary. See also ovarian cyst removal surgery.
- Surgery on the bowel or bladder: when endometriosis has grown into these organs, part of the organ may need to be removed. This is complex surgery and may need a specialist team.
- Hysterectomy, with or without removal of the ovaries: the womb, and sometimes the ovaries, are removed. It is a last-step option. Read about it on our hysterectomy page.
Where the disease is complex or deep, you may be referred to a specialist endometriosis service. Dr. Kavitha will say so openly if your case needs one.
Keyhole Surgery for Endometriosis
Most endometriosis operations are planned as laparoscopic surgery. Small cuts mean less pain after the operation, a shorter hospital stay and a faster return to normal life than an open cut. Sometimes the surgeon has to convert to an open operation if the disease is more extensive than the scans suggested, and this is discussed in advance.
Surgery and Fertility
Endometriosis can make it harder to get pregnant. Removing disease may improve the chances for some women, but the benefit is not the same for everyone. Operating on endometriomas can reduce the egg supply of the ovary, and repeated surgery adds to this. For some women, particularly those with reduced egg reserve or a long period of trying, assisted conception may be a better route than surgery. Tell your doctor at the first visit whether you want a pregnancy soon, because this changes the plan. Our infertility evaluation page explains the tests we use.
Recovery After Endometriosis Surgery
After keyhole surgery, many women go home within a day or two and return to light activity in one to two weeks. Recovery after bowel or bladder surgery or a hysterectomy takes longer. In the first weeks:
- Walk a little every day, and rest when you are tired
- Avoid heavy lifting and strenuous exercise until your doctor allows it
- Expect some shoulder-tip pain and bloating after keyhole surgery, which settle in a few days
- Ask your doctor about hormonal treatment after surgery, which is often advised to lower the chance of symptoms returning if you are not trying to conceive
Contact the clinic or go to hospital if you have fever, heavy bleeding, severe or worsening pain, a red or leaking wound, or trouble passing urine or stool.
Risks and Recurrence
Like any operation, endometriosis surgery carries risks, including bleeding, infection, a reaction to the anaesthetic, injury to the bowel, bladder or ureters, and scar tissue. The risk is higher when the disease is deep. Symptoms can return after surgery, and scarring can itself cause organs to stick together, so some women need further operations. This is why the decision to operate, and the plan for medicines afterwards, are made together.

Frequently Asked Questions
When should I consider surgery for endometriosis?
When medicines have not controlled your pain, when the disease is extensive or has formed ovarian cysts, or when it may be affecting your fertility. Mild symptoms that respond to medicines rarely need surgery.
Is endometriosis surgery done by keyhole?
Most often, yes. Keyhole surgery means smaller cuts and quicker recovery. Deep or extensive disease can need a larger operation.
Will surgery get rid of my endometriosis for good?
Not always. Surgery can reduce pain greatly, but endometriosis can return, and further surgery may be needed.
How long does it take to recover?
After keyhole surgery many women return to light activity in one to two weeks. Bowel or bladder surgery or a hysterectomy takes longer. Your doctor will give you a plan.
Can I get pregnant after endometriosis surgery?
Many women do. Surgery may improve the chances for some, but it can also affect the ovaries, so please discuss your plans with the doctor before the operation.
Will I need a hysterectomy?
Only in selected cases, usually when other treatments have failed and you do not want more children. Many women never need one.
Can endometriosis come back after surgery?
Yes. Hormonal treatment after surgery is often advised to lower the chance, unless you are trying to conceive.
Is surgery needed to diagnose endometriosis?
Not always. Doctors can often make a diagnosis from symptoms, examination and scans. A laparoscopy confirms it when the diagnosis is uncertain.
What are the risks of endometriosis surgery?
They include bleeding, infection, injury to nearby organs and scar tissue. Risks are higher when the disease is deep, and your surgeon will explain the ones that apply to you.
