Hysterectomy Surgery

Hysterectomy in Bangalore

A hysterectomy is an operation to remove the uterus (womb). It is usually advised only after less invasive treatments have been tried, for problems such as heavy periods, fibroids, long-term pelvic pain, prolapse or, less often, cancer. After a hysterectomy, periods stop and pregnancy is no longer possible. The operation can be done through the vagina, through keyhole incisions or through an abdominal cut, and the route depends on your condition. At Sai Speciality Center in Kasturi Nagar, Bengaluru, hysterectomy is planned after a full evaluation under Dr. Kavitha Lakshmi Easwaran, MBBS, MD (OBG). A second opinion is always reasonable before surgery.

Quick answers

  • Is hysterectomy major surgery? Yes. It is done under anaesthesia and needs a hospital stay and several weeks of recovery.
  • How long is recovery? Full recovery often takes around six to eight weeks, and keyhole or vaginal surgery is usually quicker than an abdominal cut.
  • Will I go into menopause? Only if the ovaries are removed, in which case menopause begins straight away. If the ovaries stay, menopause may still come a little earlier.
  • Are there alternatives? Often yes. Medicines, a hormone coil, procedures on the uterine lining, fibroid treatment or a pessary for prolapse may be tried first.
  • Do I still need cervical screening? That depends on whether the cervix was removed and why. Your doctor will tell you.

Why Is a Hysterectomy Advised?

Common reasons include:

  • Heavy or prolonged periods that have not responded to medicines or other treatment
  • Fibroids causing pain, pressure or heavy bleeding
  • Long-term pelvic pain, including some cases of endometriosis or adenomyosis
  • Uterine prolapse
  • Cancer or pre-cancerous changes of the womb, cervix or ovaries

For most benign conditions, hysterectomy is a last step rather than a first one. A good evaluation explains what else could be tried and why surgery is or is not the right choice for you.

Alternatives to Consider First

  • Medicines to reduce bleeding or pain, such as tranexamic acid, painkillers or hormonal treatment
  • A hormone-releasing coil (intrauterine system) for heavy periods
  • Treatment of the uterine lining to reduce bleeding, in women who have completed their families
  • Fibroid surgery, which removes fibroids and keeps the womb, or uterine artery embolisation
  • A pessary or pelvic floor treatment for mild prolapse

If you are still hoping to have children, tell your doctor early, because several of these options keep the womb.

Types of Hysterectomy

  • Total hysterectomy: removes the uterus and cervix. This is the most common type.
  • Subtotal (partial) hysterectomy: removes the main body of the uterus and keeps the cervix.
  • Hysterectomy with removal of the tubes and ovaries: done in selected cases, such as some cancers or severe endometriosis. Removal of both ovaries causes immediate menopause.
  • Radical hysterectomy: removes the uterus and surrounding tissue, and is used in some cancers.

Whether the ovaries are kept is a decision to make with your doctor before surgery, taking into account your age, family history and the reason for the operation.

Surgical Approaches

  • Vaginal hysterectomy: the uterus is removed through the vagina, with no abdominal cut.
  • Laparoscopic (keyhole) hysterectomy: small abdominal incisions with a camera. See our page on laparoscopic gynaecology surgery.
  • Abdominal hysterectomy: the uterus is removed through a cut in the lower abdomen. It may be needed for a very large uterus, extensive scarring or some cancers.

Where it is safe, vaginal or keyhole surgery is usually preferred because of the shorter stay and quicker recovery. Your surgeon recommends the route after examining you and reviewing your scans.

Before the Operation

  • Blood tests, including a haemoglobin check, because heavy bleeding often causes anaemia
  • Ultrasound or other scans, and cervical screening if due
  • A review of medicines, diabetes, blood pressure and other health conditions
  • A discussion of the risks, the ovaries and what to expect after surgery
  • Anaesthesia assessment

Recovery After a Hysterectomy

The hospital stay is usually a few days, depending on the type of surgery. Full recovery often takes around six to eight weeks, and some women feel back to normal sooner after keyhole or vaginal surgery. In the first weeks:

  • Walk a little every day, and rest when you are tired
  • Avoid heavy lifting and strenuous exercise until your doctor allows it
  • Keep follow-up appointments
  • Ask your doctor when it is safe to drive and to resume sex

Contact the clinic or go to hospital if you have fever, heavy bleeding, severe or worsening pain, swelling or discharge from a wound, or difficulty passing urine.

Risks and Effects

Hysterectomy is generally safe, but like any operation it carries risks, including heavy bleeding, infection, injury to the bladder or bowel, blood clots and a reaction to the anaesthetic. Your surgeon will discuss the risks that apply to you.

After a hysterectomy:

  • Periods stop
  • You can no longer become pregnant
  • If the ovaries are removed, menopause begins immediately, and hormone replacement therapy can be discussed. See our menopause care page
  • Some women notice a change in mood or sexual feelings, and many feel relief from long-standing pain or bleeding

Frequently Asked Questions

Is hysterectomy a major surgery?

Yes. It is done under anaesthesia and needs a hospital stay and several weeks of recovery, although keyhole and vaginal routes make recovery easier.

How long does it take to recover from a hysterectomy?

Full recovery often takes around six to eight weeks. Keyhole or vaginal surgery is usually quicker than an abdominal cut, and your doctor will guide you on activity.

Will I need to stay long in hospital?

The stay is usually a few days. It depends on the type of surgery and how you recover.

Will a hysterectomy cause menopause?

Only if both ovaries are removed, which brings on menopause straight away. If the ovaries are kept, menopause may still come slightly earlier than it otherwise would.

Can I get pregnant after a hysterectomy?

No. Once the uterus is removed, pregnancy is not possible.

Are there alternatives to hysterectomy?

Often yes, depending on the condition. They include medicines, a hormone coil, fibroid treatment, treatment of the uterine lining and a pessary for prolapse.

Do I still need Pap smears after a hysterectomy?

It depends on whether your cervix was removed and why the surgery was done. Ask your doctor before stopping screening.

Will a hysterectomy affect my sex life?

Most women can resume sex once healed, usually after the weeks your doctor advises. Some notice changes, and many feel better once pain or heavy bleeding has gone.

Should I get a second opinion before a hysterectomy?

It is reasonable to, especially for a benign condition. A second opinion can confirm the diagnosis and whether other treatments are worth trying first.
